Patient Forms

Prior to the Consultation visit with Dr. Engelbart you will be mailed or presented with a documentation packet to complete.  It will consist of the usual privacy forms, personal contact and health history forms.  In addition you will find forms that ask additional health questions that are pertinent to your case.

We do not release or request any information without your written approval.

Consultation

Next you will have a consultation with Dr. Engelbart to discuss your health-related problems and concerns, as well as potential treatment options.

• This visit provides you with the opportunity to "interview the staff", "evaluate the office environment".  This is important, you want to feel safe, welcomed and respected by everyone you have contact with.
• Your first visit is designed for the doctor to learn more about you, your condition and your expectations and to determine whether functional neurology, clinical nutrition or chiropractic care will help you reach your goals.

Examination

If it has been determined that Dr. Engelbart's expertise and knowledge will help you reach the goals a visit will be scheduled for an examinantion.  Dr. Engelbart will perform a complete chiropractic and neurological examination.

Dr. Engelbart may recommend that labs be done on patients having metabolic or neuro transmitter issues.  She will generate the order, and in some cases, ask that they be completed prior to the next visit. 

Important Note:  WE WILL NOT PERFORM ANY TREATMENT WITHOUT A THOROUGH EXAM. You should never let  any doctor treat or adjust you without examining you first.

Imaging Studies

We do not offer any imaging on site.  If it is determined that your specific condition requires x-rays or another form of imaging we will generate an order so that you may schedule at a location of your liking.  Imaging helps us rule out more serious conditions or assist us in developing the most effective treatment plan for you.

Report of Findings

After all required information is gathered and the examination is completed Dr. Engelbart will review and study your case.  She will put together in report form the Report of Findings.  It will include:

• The findings
• The recomendations
• Timelines for treatment
• Other, i.e.: additional recommended testing, supplementation requirements

She will sit and discuss the report at length with you, one-on-one, in this extended visit.  Any questions you may have regarding the care, cost, time, expected outcome, etc. is open for discussion.  Dr. Engelbart uses this time to educate you.

Treatment

Patients requiring treatment will generally receive their first treatment at the conclusion of the Report of Findings - after they are satisfied with the plan presented.

• This may include spinal adjustments, physical therapies, neuromuscular re-education and/or soft tissue therapies. Treatment is interactive with you so you can express any concerns that you may have.
• If specific supplement or other nutrition protocal is needed the recomendations are presented  and the prescribed items offered to you at this visit for purchase.  
• In this visit, patients may be advised and given a program to be conducted at home. If you are in pain when you first come into our office this may include avoidance of certain activities or positions, as well as home exercises and/or stretches.  Dr. Engelbart may prescribe exercises affecting your nervous system to help you attain your goals.

From here on out the process is a TEAM effort.  YOU will be responsible for keeping up with the protocal defined  for you, doing your exercises, keeping your appointments, supplementing  as directed, staying away from anything harmful to your goals!

The more you follow Dr. Engelbart's recomendations the quicker you will make the progress you want.  We will always give you our best but... you have to do the work.
